2. ON LINE using the "submit"
button below. All registrations will be submitted via a secure server. Be
sure to include your credit card information.
3. PRINT AND MAIL this
registration form, along with your check or credit card information to 133

4. PRINT AND FAX this
registration along with your credit card information to 703-242-1454.
Registration closes at noon
the day before the program. NO registrations will be processed after that
time
CANCELLATIONS: If you
register for a program and find you must cancel, please notify the Center
in writing at least five days in advance of the program in order to receive
FULL CREDIT. No credit if less than 5 days notice is given. FULL REFUNDS
are given for any programs cancelled by the Center. If you are unable to
attend a program that is rescheduled, you qualify for a full refund as well.

ext. 397. SPECIAL ASSISTANCE: If you require special assistance because of
physical limitations, please notify the Center at least 14 days in advance
of the program.
